All of our rehabilitation, physiotherapy and hydrotherapy sessions, including fun and fitness swims, require veterinary referral from your primary care veterinary surgery. You can start the referral process by completing and submitting this form.
Please note we will contact your primary care veterinary practice to obtain their permission for us to provide this service. In doing this we will also request a full clinical history for your pet
Once you have submitted this form and we have a returned veterinary referral form, a member of our team will contact you directly to make your first appointment.
